The ender portal is a structure that allows the player to move between the End and the Overworld.
Generation
The ender portal is made of seven ender portal frames in a row. A few of them may already contain an eye of ender.
Skyhold
The ender portal generates in the overworld in the skyhold. It is usually so high up that the sky begins to turn dark.
The End
When the player defeats the ender dragon, a chest containing loot and an already activated ender portal are spawned. This portal takes the player back to the Overworld and when the player is teleporting back to the Overworld a piece of text is scrolled through in front of a stone brick background.
