Use
Shift-clicking an ender pearl, it will cause you to throw it. It will teleport you to where it lands and will take 2 and 1 half hearts of your health.
It is used to craft an Eye of ender to get to the sky hold and defeat the ender dragon or teleport.
To teleport you will need to shift-click with your selection on a ender pearl.

It was a rare item dropped by the Enderman.
Trivia
Since the 1.2.6 update, the drop rate is boosted.
